Passport Application Guidelines
Applicants are requested to send their passport applications by courier to the following address:

Alternatively, applicants can request an appointment using the abovementioned email address, in order to submit their passport applications by hand during the following Consular working hours:
Consular working hours: 9:30 am to 13.30 pm from Monday to Friday.
Procedure
Please be advised as follows:
• The old Guyana passports are no longer being renewed with effect from December 2014. In light of this, a new machine readable passport will be needed for holders of the old Guyana passports.

• When an application is made at the High Commission, it has to be forwarded to the authorities in Guyana for processing. Once the application process is completed and the application approved, the new Passport will be forwarded to the High Commission for issuance to the applicant.
• Application forms must be completed by the applicant in duplicate.
Passport Application Form
• Failure to complete the form properly; as well as to provide supporting documents, would result in applications being rejected or delay the processing of the application.
Requirements:
• The following documents are to be submitted when applying for a new passport:
First Time Applicants:-
– Completed application form in duplicate (endorsed by the Recommender, as required on page 3 of the application form: Section 7: Reference.)
– 2 coloured passport-size photographs of applicant on white background (endorsed by the Recommender) – Please do not let the Recommender stamp at the back of the photographs and do not use staples or paper clips on the photographs. Kindly place the endorsed photographs in a small envelope and submit with the application form and supporting documents.
– Original Birth Certificate or notarized copy of the Birth Certificate. – Photo identification (Identification Card, Driver’s Licence, etc.). – Notarized copy of Adoption Certificate (if adopted).
– Notarized copy of Certificate of Naturalization/Registration as a Guyanese, if applicable.
– Legible signature in the designated box at the top of page 2 of the application form and in the designated space on page 3, Section 6: Declaration. – Name change documents (married certificate, deed poll, etc.) – Original or notarized copies.
Passport Renewal:-
– Completed application form in duplicate (endorsed by the Recommender, as required on page 3 of the application form: Section 7: Reference.)
– 2 coloured passport-size photographs of applicant on white background (endorsed by the Recommender) – Please do not let the Recommender stamp at the back of the photographs and do not use staples or paper clips on the photographs. Kindly place the endorsed photographs in a small envelope and submit with the application form and supporting documents.
– Name change documents (married certificate, deed poll, etc.) – Original or notarized copies.
– Existing Guyana passport or two (2) notarized copies of the passport bio-data page.
– Legible signature in the designated box at the top of page 2 of the application form and in the designated space on page 3, Section 6: Declaration.
– In the case of a lost or damaged passport, a Police Report, Affidavit and original Birth Certificate needs to be submitted with the application.Lost Passport Form
Children under the Age of 18:-
– Completed application form in duplicate (endorsed by the Recommender, as required on page 3 of the application form: Section 7: Reference.)
– Parent’s photo identification (Identification Card, Driver’s Licence or bio-data page of valid passport.)
– Original birth certificate in case of first time applicant.
– Notarized copy of Certificate of Registration as a Guyanese, if applicable, in case of first time applicant.
– Existing Guyana passport or two (2) notarized copies of the passport bio-data page, in case of renewal.
– Parent needs to sign page 3 (upper part) of the application form.
– 2 coloured passport-size photographs of applicant on white background (endorsed by the Recommender) – Please do not let the Recommender stamp at the back of the photographs and do not use staples or paper clips on the photographs. Kindly place the endorsed photographs in a small envelope and submit with the application form and supporting documents.
– Legible signature in the designated box at the top of page 2 of the application form and in the designated space on page 3, Section 6: Declaration.
– Children 10 years and older are required to sign on their application form.
Note:
✓ All applicants are required to submit their Proof of Payment (Deposit Slip) with their passport application.
✓ An applicant who is applying for the first time or who has the old Guyana Passport (Green Book) and do not have any form of photo identification can be identified by their nearest relative such as their mother, father, brother, sister, husband or wife with a copy of their Identification Card
and birth certificate. If it is the Recommender identifying the applicant, he/she would have to provide a Statutory Declaration and copy of their Identification Card or bio-data page of their passport.
Passport Application Fees:-
– South African Rand 1,600.00 for Nationals residing in South Africa. For nationals residing out of South Africa the fee is US$125.00.
– Lost Passport Fee – South African Rand 2,800 for Nationals residing in South Africa. For Nationals residing out of South Africa the fee is US$225.
Please note the abovementioned fees exclude Bank charges, which is the responsibility of the applicants. This means that you will have to pay all the bank charges for the transaction as well. We need to receive the full amount for the passport fee. PERSONAL CHEQUES ARE NOT ACCEPTED.
